DAY 1

  • Review NDIS expectations and the latest legislative updates when conducting investigations in disability care  

  • Exploring the key principles and ethical considerations of investigations  

  • Identifying the different types and scope of investigations  

  • Identifying an incident VS a near miss   

​

Session 2: Planning and Preparation for Investigations – Tools and Tactics   

​

  • Developing a comprehensive investigation plan  

  • Identifying and gathering relevant evidence  

  • Establishing a timeline and resources for the investigation process  

  • Setting expectations of stakeholders  

Practical Activity:
Drafting an investigation plan based on an example investigation scenario

Session 3: Interviewing Techniques and Skills

  • Refining investigative interview techniques and personal style  

  • Strengthening communication and observation skills  

  • Handling challenging interview situations

Session 4: Analysing Evidence and Making Findings  

​

  • Evaluating and assessing gathered evidence  

  • Applying logical reasoning, root cause analysis and critical thinking  

  • Formulating clear and concise findings  

Practical Activity:
Analysing a set of evidence and formulating findings based on the provided investigation case

  • Learn how to structure and format investigation reports effectively 

  • Focus on writing reports that are clear, objective, and unbiased 

  • Ensure all relevant details and supporting evidence are included

Defensible Documentation 

  • Understand the importance of accurate and comprehensive documentation in protecting against legal and compliance risks 

  • Identify key elements of defensible documentation, including accuracy, completeness, and timeliness 

  • Follow best practices for documentation, such as using standardised formats, avoiding subjective language, and securing records 

Practical Activity:
Participants will draft an investigation report based on a provided case and review sample documentation to identify strengths and areas for improvement in defensibility.

Session 6: Ensuring Fairness and Procedural Fairness

  • Understanding the principles of procedural fairness  

  • Recognising the importance of impartiality and neutrality in investigations  

  • Addressing conflicts of interest and bias during the investigation process

Session 7: Managing Confidentiality and Privacy

  • Respecting privacy and confidentiality in investigations  

  • Safeguarding sensitive information and personal data  

  • Adhering to relevant legislation and organisational policies

Session 8: Finalising Investigations and Follow-up Actions

  • Formulating recommendations and corrective actions
